Texas Ophthalmological Association Medicare Part D Drug Search BETA TESTING Terms and Conditions This list of partial formularies for common ophthalmic drugs under Medicare Part D in Texas and is not meant as an endorsement of any of the drug plans or drugs which appear herein. The source of the data is the CMS Formulary Finder. TOA shall not be held liable any decision made or action taken by you in reliance on such information. This list may not be duplicated, sold, retransmitted, reposted, or otherwise used for commercial purposes. By clicking "Submit Query" below, you indicate your understanding of and agreement to these terms and conditions. The above linked CMS Formulary Finder allows you to enter drugs and search for plans whose formularies include those drugs. That's great for patients but no good for providers. The TOA Drug Search reverses the process by allowing you to select a patient's drug plan and then listing whether common ophthalmic drugs are on that plan's formulary. This search function is in beta testing.
